By booking an appointment, you agree to the following terms and conditions:
1. Scope of Services
We provide services in physiotherapy, acupuncture, and natural medicine. Treatments are tailored to the individual and delivered by qualified and experienced practitioners.
2. Appointments
All sessions are by prior appointment. Please arrive on time to ensure you receive the full duration of your treatment.
3. Cancellations and Rescheduling
If you need to cancel or reschedule your appointment, we kindly ask for at least 24 hours’ notice:
Cancellations or changes made with less than 24 hours’ notice will incur a £30 fee.
Missed appointments without notice (no-shows) will also be charged £30.
4. Deposits
A £30 deposit may be required at the time of booking to secure your appointment. This will be deducted from the final treatment fee.
The deposit is non-refundable in cases of late cancellation or missed appointments.
5. Payments
Payment is due in full at the time of the appointment unless otherwise agreed in advance. We accept cash, bank transfers, and selected card payments.
6. Client Consent
All clients are required to give consent for both the processing of their personal data and for treatment. By signing the Initial Assessment Form, clients confirm that they:
Consent to the processing of their medical and personal information for treatment and administrative purposes.
Consent to receive treatment from the attending practitioner.
Consent to the sharing of relevant clinical information with their GP, consultant, or referring medical professional upon request.
7. Confidentiality
We maintain strict confidentiality in line with professional and legal standards. This means:
Access to personal or medical data is restricted to those who need it for legitimate treatment or administrative purposes.
Personal information is only used when absolutely necessary and lawful.
Everyone accessing client information is fully aware of their responsibilities and bound by confidentiality agreements.
Your data is securely stored and handled in accordance with current data protection regulations.
8. GDPR and Data Access
This practice complies with the General Data Protection Regulation (GDPR) 2018, which governs how personal information is handled, both digitally and in paper format.
Clients have the right to access the information held about them.
Requests must be submitted via email. A Release of Medical Records Form will need to be signed.
An administrative fee of £50 applies for the medical reports.
9. Health Information
Clients are responsible for providing accurate and up-to-date health information. It is essential to inform your practitioner of any medical conditions, medications, allergies, or significant changes to your health prior to each session.
10. Right to Refuse Treatment
We reserve the right to decline or discontinue treatment if it is not deemed in the client’s best interest, or if practice’s policies are not respected.
